Prescriptions were misplaced by Leeds GP surgery

The Care Quality Commission (CQC) told The Highfield Medical Centre on Highfield Road that it required improvement to its safety and leadership.

The report said: “There had been a number of significant events where prescriptions had been misprinted or lost. At the time of our visit we were unable to see any evidence of how these were documented or recorded to keep a log of unused prescriptions.

“Staff understood their responsibilities to raise concerns, and to report incidents and near misses. However, when things went wrong reviews and investigations were not thorough enough and lessons learned were not communicated widely enough to support improvement. Patients did not always receive a verbal and written apology.”

One member of staff had been appointed without references having been received.

Inspectors also reviewed three sets of patient records and found no advice had been recorded. And records were not maintained to support the investigations and findings resulting from complaints the practice had received in all cases.

But the watchdog said following its inspection the practice has shown evidence that a new system of checking and logging was in place.

Highfield was rated “good” in the categories of whether the service was effective, caring and responsive to people’s needs.

The report said staff had the skills, knowledge and experience to deliver effective care and treatment.

And two patients interviewed said “they were satisfied with the care provided by the practice and said their dignity and privacy was respected”.
