After the operation, the patient was transferred onto the ward and given one-to-one supervision, but two hours later they were found to have suffered a respiratory arrest and subsequently died.

The report to directors of Leeds Teaching Hospitals NHS Trust says the police and Leeds City Council’s social services department were informed. An investigation is taking place.

Other incidents in January and February included a surgical error affecting a patient having a wound washed out and the wrong lens being implanted during a cataracts operation.

During the two months there were also six serious pressure ulcers and six falls in patients reported.
